Improve Your Agency's OASIS Scores

The Purpose of OASIS in Home Health Care:

The Outcome and Assessment Information Set (OASIS) is a key component of Medicare’s partnership with the home care industry. OASIS is designed to foster and monitor improved home health care outcomes, and it is an integral part of the Conditions of Participation for Medicare-certified home health agencies (HHAs). Outcomes derived from OASIS items have a very specific definition: they measure changes in a patient’s health status between two or more time points. The data points are collected at start of care, 60-day follow-ups, discharge and surrounding an inpatient facility stay.

In simple terms, the OASIS is the key factor in determining agency reimbursement and the improvement in the patient’s condition under agency care.

Why Should OASIS Scores Be Top-Of-Mind?

OASIS data is a major contributor to the agency quality measures – or scores – that are visible on the CMS Home Health Compare website. Home Health Compare shows the public how often each home health agency used best practices when caring for its patients, and whether patients improved in certain important areas of care. Your agency’s good outcomes can attract new patients as well as potential employees.

How Shield HealthCare Can Help:

Home Health Agency scores come directly from comparing the OASIS assessment on patient admission to the OASIS assessment on patient discharge. For this reason, it is important for clinicians to score patients as accurately as possible at the start of care, and to support their patients with case-specific health care products that improve patient outcomes.
