Wound Care Part 2: Assessment and Documentation

When treating a wound, it is important to take a holistic approach to identify factors that can help or impeded wound healing.

A complete medical history including family history, current illnesses, procedures, allergies and nutrition will not only help you build rapport with your patient but also help you provide the more appropriate care. In addition, understanding their psychosocial history will help you determine if your patient can take care of themselves.

Finally, before a wound evaluation, a physical evaluation should be done to consider how their conditions can impact the healing process. Then a complete wound evaluation can be done in a systematic manner with all pertinent information documented with as much detail as possible.

Learning Objectives 

After completing this course, the learner should be able to:

  • Describe the importance of performing a medical and psychosocial history, and be able to do so
  • Perform a complete physical exam of a patient with a wound
  • Perform a complete wound evaluation, including an assessment of the wound base and surrounding tissues
  • Assess and monitor wound complications
